# Sesami Forms-App

Erstellen Sie benutzerdefinierte Aufnahmeformulare, um während des Buchungsprozesses Informationen von Ihren Kunden zu erfassen.

## Was Sie tun können

* Benutzerdefinierte Fragen und Felder zur Buchungsseite hinzufügen
* Felder als erforderlich oder optional markieren
* Informationen wie Krankengeschichte, Vorlieben, Allergien oder Sonderwünsche erfassen
* Die erfassten Daten zur Vorbereitung auf Termine verwenden

Die Sesami Forms App ist verfügbar über das **Sesami-Admin-Portal** und funktioniert mit **Sesami Sofortbuchung** nur.

## Formulare einrichten

1. Gehen Sie zu **Sesami Admin Portal > Apps**
2. Installieren Sie die **Sesami Forms App**
3. Im **Formulare** Bereich erstellen Sie Ihr Formular:
   * Einen Formularnamen hinzufügen
   * Aktivieren Sie es
   * Die benötigten Eingabefelder und Fragen hinzufügen
4. Das Formular mit einem Service verknüpfen

## Anwendungsfälle

* **Salons** — nach Haartyp, Allergien oder Stilvorlieben fragen
* **Wellness** — Gesundheitsdaten vor einer Sitzung erfassen
* **Beratungen** — Hintergrundinformationen sammeln, damit Sie sich vorbereiten können
* **Veranstaltungen** — nach diätetischen Einschränkungen oder Barrierefreiheitsanforderungen fragen
* **Einwilligungen** — fügen Sie ein Kontrollkästchen zur Akzeptanz der Geschäftsbedingungen hinzu (siehe [Einverständniserklärung & Bedingungen](/de/buchung/booking-features/waiver-terms.md))
